What is an immigration medical exam and why do I need to complete one?
Generally, all applicants filing for immigrant visas and some non-immigrant visas, refugees and adjustments of status to that of a permanent resident, must submit a form I-693 medical exam completed by a designated Civil surgeon. The form I-693 medical exam is used to report results of a medical examination to USCIS. The examination is required to establish that you are not inadmissible to the United Status on public health grounds.
Who is a Civil surgeon?
A Civil surgeon is a licensed and experienced doctor (Physician) approved/authorized by the USCIS to conducted immigration medical exam.
Oakton Health Center has Civil surgeon on duty six days a week. We are always prepared and ready to provide the service.
Can the applicant have his/her or any physicians perform the medical examination?
Only the licensed and experienced doctors approved by the USCIS, who are familiar with immigration medical policy and updates can perform this exam. Usually called Civil Surgeon (In the U.S) or Panel Doctor (Outside of the U.S).
Which form needs to be submitted for the immigration medical exam?
This depends on where the applicant is applying for a Medical Exam. There are two different forms needed by the applicant filing for adjustment of status to that of a lawful permanent resident depending on where he/she applying from.
Applicants applying in the United States have to submit form I-693 completed by a designated civil surgeon. Or, applicants applying outside the United States have to submit form DS-2053 (Formerly form OF-157) given by the U.S. Consular.
Oakton Health Center will provide you the respective form.
What Documents are required for the Medical Exam?
Please bring your passport or drivers license or state ID to the appointment.
Nice to have:
- Vaccinations records
- Alien Registration Number (9-digit USCIS number or A-Number)
What happens after the medical exam?
After the medical exam is complete, the Civil surgeon will certify the results on the appropriate forms and place them in a sealed envelope with label on the back – “DO NOT OPEN THE SEALED ENVELOPE.”
Do not accept it if it is not in a sealed envelope. USCIS will return the form to you if it is not in an envelope or if the envelope has been opened or altered.
You, the applicant, not the Civil surgeon, are required to submit the Form I-693 to USCIS. You need to turn in your envelope with your immigration application.
Follow the instructions on or included with the application or the instructions given to you by the office requesting the medical examination.
How long is my medical exam valid?
Applicant is required to submit a form I-693 that is signed by a civil surgeon no more than 60 days before filing the underlying application for an immigration benefit. Form I-693 would remain valid for a two-year period following the date the civil surgeon signed it. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide for sufficient time for the performance of laboratory testing.
